Hudson Headwaters Anticipates 2024 Opening of New PACE Program

 

GLENS FALLS - The first Program of All-Inclusive Care for the Elderly (PACE) program in our region, PACE at Hudson Headwaters, is under development with plans to open in early 2024. PACE is a nursing home alternative for eligible individuals aged 55 and older who need assistance in their day-to-day living.

PACE at Hudson Headwaters will include a new, state-of-the-art PACE Day Center in Glens Falls that will serve as the hub for socialization, therapeutic activities and medical appointments for participants. PACE programs provide a wide range of services, including primary and specialty medical care, prescription drug coverage, preventive care, rehabilitation, social services, occupational and physical therapy and adult day programs. This multidisciplinary approach ensures that all aspects of an individual’s health and well-being are addressed, promoting better health outcomes and an increased quality of life.

The new PACE program in Glens Falls will be one of many federal and state-funded PACE programs in the United States. PACE was established in 1971 as a response to the growing need to care for an aging population in California. Today, there are more than 150 PACE programs across the country that serve more than 66,000 participants per year. Since its inception, PACE has made significant strides in transforming elderly care by promoting a person-centered approach and addressing the holistic needs of seniors.

The PACE model offers numerous benefits for both participants and their families. Participants receive personalized care plans tailored to their unique needs. Current PACE programs report that participants benefit from reduced hospitalizations and improved functional abilities. At the same time, caregivers are supported through training, support groups and respite care to help keep their loved ones in the community for as long as possible. The program also helps to alleviate the financial burden of long-term care, as it covers a wide range of services, including medications and medical equipment, without additional out-of-pocket costs.

PACE at Hudson Headwaters is located adjacent to Hudson Headwaters Glens Falls Family Health Center at 38 Larose Street, Glens Falls, NY and will serve select zip codes within Saratoga, Warren and Washington counties.
